2023-24 Online Pre-Participations Forms (Mandatory for ALL Athletes)
Memorial High School Athletics partners with Rank One Sport for our online paperwork needs. We will begin accepting new forms now for the 2023-24 school year. All athletes are required to complete these forms before the first day of school so that they will be eligible to participate in practices with the exception of Fall Sports (prior to July 31).
You may upload your Signed Medical History and Physical via this link. Make sure it is in PDF format. New this year Rankone will allow for uploading your physical front and back into Rankone directly. However, please make sure it is filled out completely and signed by both a parent and student. As always keep a copy for your records incase it is not completed correctly.
This will need to be the most current version of the 2020 Physical Form or from Rank One. (Do not rely on the Medical Office to have the correct form. You will be required to redo it if not correct).
Optional item is Electrocardiogram (ECG) on the PRE-PARTICIPATION PHYSICAL EVALUATION (PPE) form.

All athletes will need to have a baseline ImPACT test done every 2 years prior to the start of the school year. We will be testing your child during the athletic period at the end of this school year to ensure that all athletes get the test. As a parent you do not need to do anything, just rest assured that we will take care of this and we are looking out for the best interest of your child.

Concussions
Should your athlete sustain a concussion, they will then be held to the FISD Concussion Management Protocol and Return to Play Guidelines. That document can be found by clicking on this link below. In addition, they will be asked to complete the Daily Symptom Checklist form each day before and after school.

Injury Protocol
Student Athletes must report their injuries to the Athletic Trainer if not at the time of injury then the next school day.

Cardiac Screening
FISD Athletic Department cares about your child's heart health. In 2020 the UIL has updated their PPE form to include the option for the student to have an Electrocardiogram (ECG) test performed on their heart at the family’s own expense.
You have two options concerning the UIL PPE form:
- According to the American Heart Association, an ECG gives two major kinds of information.
- It measures time intervals of the heart on the ECG showing normal or irregular intervals.
- It measures the amount of electrical activity of the heart muscle on the ECG showing normal or overworked heart muscle.
- It measures time intervals of the heart on the ECG showing normal or irregular intervals.
You have two options concerning the UIL PPE form:
- The first option is to decline the ECG and complete the physical as it has been done in the past. If you choose this option, simply don’t check the box.
- The second option is to choose the ECG and complete the physical as it has been done in the past. If you choose this option, simply check the box. This testing will be at the Parent/Guardian expense. Results must be returned to an athletic trainer. Consult with an athletic trainer if assistance is needed.
